St. Peter's Health Partners Hospice is seeking a per diem Registered Nurse to join our Hospice Home Care Team, serving families in the Albany/Rensselaer County region. Duties include coordinating with an interdisciplinary team to develop care plans for patients at home, with a weekday schedule (Mon–Fri) and hours 8:00 a.m.
to 4:30 p.m.; weekends and holidays are not required. Applicants must hold an active New York RN license and have at least 1 year of RN experience, preferably in Hospice/Home
