• The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN).
• Delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan.
• Maintains infection prevention and control policies and uses Personal Protective Equipment as required.
• Assists with basic health care needs, including bathing, toileting, grooming, dressing/undressing, vital signs, and psychosocial support.
• Assists with transferring, repositioning, walking, and uses correct transfer techniques and equipment.
• Provides range of motion and passive exercises as needed.
• Assists with food preparation and dining to create a positive dining experience; assists residents with meals and snacks, and provides water and supplements as appropriate.
• Supports providers and nursing staff with procedures when needed; documents resident interactions and outcomes related to care, behavior, activity, and dietary changes.
• Reports changing resident condition to the charge nurse and participates in wellness and recreational programs.
• May participate in departmental or organizational committees as needed.

• High school graduate or GED preferred, but not required.
• Minimum age of 16; prior clinical or nursing assistant experience in long-term care preferred (6 months preferred, 3 months required).
• Mandatory Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state-certified CNA in process within four months of start; CNA training and certification provided by the facility.
• QMAP certification may be required in Colorado facilities.
• Annual competency testing and continuing education: minimum 12 hours per year; maintain required certifications.
• If applicable, may be registered with the Unlicensed Assistive Person (UAP) Registry and may be classified as Sanford Category II Driver where required.
