Early Childhood & Family Support Leader | Building Strong Foundations Through Home Visiting, Community Partnerships & Nonprofit Program Management

Role Summary

The Home Visitor works directly with pregnant individuals and families with infants and toddlers to support healthy pregnancies, strong parenting skills, and positive child development. Success in this role looks like building trusting relationships, delivering meaningful education during weekly home visits, and helping families connect to the resources they need to thrive. This position is a key part of the organization’s early childhood and family support services, working closely with internal teams and community partners to strengthen outcomes for children and caregivers.

Responsibilities

  • Conduct weekly home visits with pregnant individuals and families with infants or toddlers
  • Build and maintain trusting, confidential relationships with parents and families
  • Partner with parents to plan and deliver education on pregnancy, prenatal care, fetal development, childbirth, and postpartum care
  • Provide guidance on infant and toddler care, child development, and strategies that support learning and growth
  • Support families with education related to health, nutrition, mental well-being, and household management
  • Assess parent and child development to individualize education and identify the need for referrals or formal evaluations
  • Help parents learn how to observe, evaluate, and support their child’s development
  • Create and implement individualized lesson plans that reflect each family’s strengths, goals, and needs
  • Encourage family participation in workshops, socialization opportunities, and center events
  • Maintain communication with community providers regarding treatment plans, progress, and follow-up needs
  • Provide crisis intervention and ongoing advocacy when needed
  • Maintain accurate, confidential family files and complete required documentation
  • Participate in trainings, staff meetings, and team collaboration
  • Uphold confidentiality and professional standards at all times
  • Perform other duties as assigned

Qualifications

  • Experience working with pregnant individuals, infants, toddlers, or families
  • Strong relationship-building and communication skills
  • Ability to provide education in a supportive, culturally responsive way
  • Knowledge of child development, parenting practices, and family support services
  • Strong organizational and documentation skills
  • Ability to work independently while collaborating as part of a team
  • Commitment to confidentiality, professionalism, and family-centered care
