Will provide direct support and coordination of services to children, youth, and families, ensuring the implementation of the High Fidelity Wraparound (HFW) model. Collaborate with families, service providers, and community partners to develop and implement individualized, strengths-based care plans.
Additional Areas of Responsibility:
Build collaborative and trusting relationships with families, empowering them to actively participate in their care planning and decision making processes. Guide families through the four phases of the HFW process (Engagement, Planning, Implementation, and Transition), ensuring fidelity to the model. Facilitate regular Child and Family Team (CFT) meetings, ensuring that all voices are heard and respected.
Other Information/Requirements:
Preferred:
Bachelor’s degree in Social Work, Psychology, or a related field with a minimum of two (2) years lived and/or paid experience working with the target population. Associate Degree in social services, human services, or an equivalent field with a minimum of four (4) years lived and/or paid experience working with the target population; or High School Diploma or General Educational Development (GED) with a minimum of six (6) years lived and/or paid experience working with the target population.
