Community Health Worker (CHW)/Certified Peer Support Worker (CPSW) (Street Medicine)

Join Wellness Equity Alliance (WEA) as a highly motivated field team lead, managing a dedicated Street Medicine team of Community Health Workers (CHWs) and Certified Peer Support Workers (CPSWs) to deliver equitable healthcare to underserved communities.

WEA is a national public health organization focused on underrepresented communities, addressing health‑care disparities and social determinants of health (SDoH). Our Street Medicine program provides behavioral health and primary care, including sexual health and gender‑affirming care, in mobile units across the county.

• Manage daily site/program operations, drive the rig, and plan team efforts.
• Utilize technology to monitor care activities and maintain electronic participant files.
• Provide data for program evaluation through KPIs and document participant encounters for monthly reporting.
• Conduct outreach activities and engage local businesses, schools, and organizations to promote services.
• Provide health coaching, patient navigation, and health education using motivational interviewing, trauma‑informed care, and harm‑reduction approaches.
• Coordinate care by identifying barriers, engaging the care team, and assisting students in accessing health services and community resources.
• Lead, mentor, and support CHWs and CPSWs, ensuring effective communication with schools, staff, and students.
• Serve as a certified Presumptive Eligibility (PE) Determiner for Medicaid, conducting screenings and completing enrollment forms in compliance with New Mexico Human Services Department guidelines.

• Certification as a Community Health Worker (CHW) and/or Certified Peer Support Worker (CPSW).
• Experience in behavioral health or community health work.
• Strong interpersonal and communication skills with the ability to build trusting relationships.
• Knowledge of community resources and experience assisting students in accessing services.
• Proficiency recording data for program evaluation and performance improvement.
• Commitment to delivering equitable healthcare and addressing SDoH.

• Bilingual (Spanish preferred).
• Personal or professional experience with the student population served, especially in areas such as mental health, substance use, homelessness, foster care, incarceration, military service, disability, or domestic violence.
• Familiarity with the geographic communities served by the mobile health clinic.
• Completion of New Mexico Human Services Department-approved Presumptive Eligibility Determiner training and certification (or ability to obtain within 90 days of hire).
• Knowledge of New Mexico Medicaid eligibility rules and enrollment systems such as Yes
