Responsibilities

• Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.
• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

• Posted Salary Range USD $20.00 - USD $27.00 /Hr.
