Center Well Home Health, part of Humana Inc., is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the guidance of a Physical Therapist. You will implement therapy plans, perform therapeutic exercises and modalities, and document care while coordinating with the clinical team.
Requirements include an active state PT license, CPR, a valid driver's license, and strong English communication. Prior experience with geriatric patients and teamwork is preferred.
