LVN​/LPN – LTC​/SNF

Job Summary:

Provide quality nursing care to residents in a long-term care or skilled nursing facility setting. Ensure patient safety, support clinical operations, and collaborate with the healthcare team to deliver effective, compassionate care.

Responsibilities:

• Administer medication and treatments accurately according to physician orders.
• Monitor and record vital signs, patient symptoms, and progress.
• Assist residents with activities of daily living, maintaining dignity and comfort.
• Collaborate with interdisciplinary team to develop and update care plans.
• Adhere to facility policies, safety protocols, and infection control procedures.

Qualifications:

• Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) credential required.
• Minimum of one year of experience in LTC/SNF settings preferred.
• Current state licensure and CPR certification.
• Strong clinical skills and compassionate patient care approach.
• Ability to work efficiently in a fast-paced environment.
