CNA – LTC​/SNF

Job Summary:

Provides essential patient care and support within long-term care or skilled nursing facility settings. Assists with daily living activities, ensuring patient comfort and safety.

Responsibilities:

• Support nursing staff by delivering direct patient care in accordance with care plans.
• Assist patients with activities of daily living, including bathing, dressing, and mobility.
• Monitor and report patient conditions and any changes to the nursing team.
• Maintain a clean, safe, and comfortable environment for residents.
• Follow infection control protocols and safety guidelines during all interactions.
• Document care provided accurately and timely.

Qualifications:

• Valid CNA certification recognized in the state of New Mexico.
• Previous experience working in long-term care or skilled nursing facilities preferred.
• Ability to communicate effectively with patients, families, and healthcare team members.
• Strong understanding of patient safety and infection control procedures.
• Demonstrate compassionate, patient-centered care and professionalism.
