• Administer prescribed medications and treatments per physician orders and facility policy
• Monitor residents’ health status, vital signs, and report changes to the supervising RN
• Perform basic nursing procedures, wound care, and specimen collection as indicated
• Document care plans and resident progress accurately in the charting system
• Assist with activities of daily living and support resident comfort and safety
• Collaborate with the interdisciplinary team to meet individualized resident goals and care plans
• Maintain infection prevention practices and ensure a clean, safe caregiving environment

• Active LPN license (state of NM) or eligibility to obtain license
• CPR/BLS certification
• 1 year of recent LPN experience in long-term care or skilled nursing preferred
• Ability to work 12-hour shifts and the 3x12 schedule
• Strong documentation, communication, and teamwork skills
• Proficiency with electronic health records and regulatory compliance
