Lily Care Hospice in Albuquerque is seeking a Hospice Clinical Liaison (RN) to build our referral network across hospitals, SNFs, and physician practices. You will educate teams on hospice eligibility and guide families to hospice at the right moment.
Based in our Albuquerque office, you will own the territory, review referrals, coordinate with the Clinical Director on complex cases, meet families to discuss goals of care, and hand off patients to the RN Case Manager while documenting in the EMR.
