Lily Care Hospice is a community-based hospice serving patients and families across central New Mexico. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

Duties and ResponsibilitiesServe as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits
Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation
Compensation and ScheduleThe Hospice Registered Nurse Case Manager role pays $90,000 – $105,000/year based on experience. Full-time, Monday–Friday day schedule with a rotating on-call assignment. Territory is based out of Albuquerque, NM.
Skills and QualificationsCurrent RN license in New Hampshire or a compact state
One year of RN experience required; hospice, home health, or case management experience preferred
Current CPR certification
Valid driver's license
