Licensed Practical Nurse (LPN)
We are seeking a skilled Licensed Practical Nurse (LPN) with previous experience in Long-Term Care (LTC) and Patient Care Coordination (PCC). The ideal candidate should be able to begin work immediately with minimal orientation provided.
Responsibilities:- Provide direct patient care in compliance with established protocols and standards.
- Coordinate with healthcare team members to ensure optimal patient outcomes.
- Monitor and document patient progress and report changes to the healthcare team.
- Assist with the implementation of care plans tailored to individual patient needs.
- Maintain a clean and safe environment for residents.
- Current and valid Licensed Practical Nurse (LPN) license.
- Prior experience in Long-Term Care (LTC) and Patient Care Coordination (PCC) is required.
- Strong communication and interpersonal skills.
- Ability to work effectively within a team.
- Willingness to work in a fast-paced environment with minimal orientation.
