Trinity Health PACE ministers exclusively to the elderly through a coordinated PACE care model which provides preventive, primary, acute and long‑term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. Many PACE participants receive most of their care from staff employed by the PACE organization in the PACE center. The primary purpose of the Social Worker position is to plan, organize, and implement social services for PACE Organization (PO) participants and families.
Responsibilities include assessment, treatment, teaching, and counseling participants, caregivers, or other appropriate representatives. Intervention may include individual participant contacts, appropriate collateral contact, participant and family education, assessment and counseling, provision of resources, addressing mental health needs, ongoing case management, advocacy to ensure participant and caregiver needs are met, and disenrollment procedures.
- Must possess a comprehensive knowledge of social work as normally obtained through a Master's degree in Social Work.
- Two (2) years of experience, including one (1) year working with the frail or elderly population.
- Current license in social work within the state of the PACE Organization preferred.
- Experience working on a multidisciplinary team in a hospital, nursing home or community‑based setting is preferred.
- Experience and thorough knowledge of social service principles and practices.
- Knowledge of financing mechanisms such as Medicare, Medicaid and other payment systems.
- Valid driver’s license and ability to provide a reliable means of transportation required.
- Position may require 10% travel within the center’s geographic region.
- Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
