Full‑Time Licensed Medical Social Worker

Location:

Alexandria, LA – St. Joseph Hospice, The Carpenter Health Network.

• Conduct patient assessments and provide ongoing care of hospice patients.
• Complete documentation accurately and in a timely manner.
• Communicate with patients, families and co‑workers.
• Manage casework needs of terminal‑ill patients and families.
• Participate in discharge planning and in‑service programs related to the needs of the patient.

• Master’s Degree in Social Work.
• Previous Home Health or Hospice experience preferred.
• Good oral and written communication skills.
• Valid driver’s license, proof of insurance and reliable transportation for in‑home visits.
