Position: HOSPICE ON-CALL NURSE - CASUAL

Responsibilities

Program Responsibilities Performs comprehensive assessment to identify the physical, emotional, psychosocial and spiritual eeds related to the terminal illness in order to promote the hospice patient s well-being, comfort and dignity throughout the dying process. Document patient assessment, interventions and plan of care in the electronic medical record including collaboration in facilities to utilize their electronic medical records. Teach end of life cares including symptom management, personal cares, medication management and administration and use of equipment to patients, caregivers, and health professionals to promote comfort through all changes at end of life.

Collaborate with hospice interdisciplinary team, physicians, pharmacies, medical equipment providers (vendors), health professionals at assisted livings, adult foster homes, skilled nursing facilities, and clinics, hospitals to coordinate and provide care. Coordinate care according to the plan of care established by the patient and family including coordination of services provided by all hospice staff, medications and equipment. Perform clinical patient care including accessing and maintenance of central lines, medication administration, and place and maintain indwelling urinary catheters, obtain specimens for lab testing, wound care, personal care and ostomy cares.

Maintain other medical equipment and devices as required. Develop a plan of care focused on the patient and family goals, implement interventions based on the plan of care, evaluate progress towards goals and adjust plan as needed. Integrates knowledge of disease process to manage symptoms working independently using hospice standing orders to problem solve, manage and titrate medications to promote comfort.

Participates in providing 24 hour on call care for hospice patients and coordinates care for patients provided by the on call team. Applies Medicare hospice regulations in practice. Provide counsel to clients and family with grief and coping with end of life changes including participation with bereavement functions. Educate health care professionals and the community at large about hospice and palliative care.

Other responsibilities as directed by supervisor.

Education and Experience

Bachelor's degree in nursing and moderate experience in community or acute care nursing. Registered Nurses with Associates/Technical degree in nursing and considerable experience in community or acute care nursing may be considered only when circumstances make it difficult or impossible to hire a bachelor s degree nurse and the Registered Nurse meets the minimum program requirements of the position for which he/she is being hired.

Physical Requirements

This is light work requiring the exertion of up to 20 pounds of force occasionally, up to 10 pounds of force frequently, and a negligible amount of force constantly to move objects. This classification involves seldom physical agility requirements such as: climbing, stooping, kneeling, crouching, crawling, reaching, pushing, pulling, repetitive motions and manual dexterity; work has standard vision requirements; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly;

hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, visual inspection involving small defects and/or small parts, using of measuring devices, operating machines, operating motor vehicles or equipment and observing general surroundings and activities;

work occasionally requires exposure to fumes or airborne particles, exposure to outdoor weather conditions, exposure to blood borne pathogens and may be required to wear specialized personal protective equipment and secondhand smoke; work is ge erally in a moderately noisy location (e.g. business office, light traffic).

Special Requirements

Current unrestricted Minnesota Registered Nurse License. If eligible, current Public Health Nurse Certification or eligible for certification within three months of hire. Upon employment or within six months of hire training in AWAIR, CPR, Employee Right to Know, NIMS, blood borne pathogens, TB/respiratory protection, Emergency Action Plan, HIPAA, Data Practices, and fire training. Valid driver's license in the State of Minnesota.
