Hiring On-Call/Casual LPNs & RNs – On-Call requires working three shifts per monthObtaining the COVID-19 vaccination, or medical or religious exemption, is a requirement of employment for this position.
ScopeResponsible for the administration of general care of the residents and patients.
Provide regular work direction and monitors the work of Nurse Assistants and Attendants on assigned shift.
Works closely with the patient/resident and with various members of other departments such as activities, meal services, social services, chaplain, housekeeping, and other members of the Nursing department.
The LPN may have contact with the family of the resident/patient.
Must have flexibility in schedule to fulfill an on-call rotation if required and be available for on‑call medical questions and/or emergencies via phone or in person as needed.
Key Responsibilities- Administer medications as doctor orders; following all physician’s orders, transcribe new orders on the medication record; administer injections, oral medications, eye drops, inhalers, all medication doses given routes as ordered, accu-checks, Foley catheters, creams, lotions, etc. to the appropriate resident/patient.
- Responsible for dressing wounds; administer oxygen, suctioning, tube feedings, etc. as ordered by physician.
- Clean and stock the med cart, med room and nurse station; count all scheduled drugs in lock box and follow all applicable state, federal, and facility guidelines in the destruction of meds.
- Obtain necessary information on resident/patient to initiate the Plan of Care per applicable guidelines.
- Monitor and provide work direction to Nurse Assistants/Registered (NA/R). May assist the NA/R with resident cares.
- Follow up with resident and NA/R to ensure proper care.
- Serve as a mentor and provide leadership and guidance in all aspects of NA/R’s performance including performance evaluation.
- Monitor the TMA in administration of all medications; provide instruction in administering nebulizer treatments, oxygen therapeutic services, glucose monitoring and Glucometer QA testing; serve as a mentor for TMA’s performance.
- Understand Resident’s Plan of Care and recognize changes; observe and document changes in physical, mental, emotional status and Activities of Daily Living.
- Observe food and fluid intake, reactions to medicine, pain, sleep patterns, vital signs, social behavior; inform physicians as warranted.
- Ensure charts, notes, and resident plan of care is up-to-date per policy and guidelines; communicate with other shifts.
- Inform resident about medicines and treatments, explain side effects and potential reactions; respect refusal.
- Explain procedures for Activities of Daily Living.
- Respond immediately to “STAT” calls; learn and know each Resident Plan of Care for life‑saving measures; administer CPR, oxygen, etc.; follow DNR/DNI/DNT instructions.
- Respond to resident incidents by observing and monitoring conditions.
- Take responsibility for welfare and safety; promote personal choices; report abuse, mistreatment, neglect; support and comfort to dying resident and family.
- Train and orient new/current employees as directed; assist in special projects; practice infection control procedures.
- Create a home‑like environment for each person we serve.

Education and Experience Required- Licensed as a Practical Nurse in the state of Minnesota.
- Obtain CPR certification within 6 months from date of hire.
- Complete and pass a state required background study.
- Maintain own educational competency.
- Strong organizational skills; ability to work with a variety of residents/patients; work effectively with co‑workers as a team and individually.
- Exceptional customer service skills and ability to work effectively with changing priorities.
- Experience working with the elderly in long‑term care, home care, or assisted living settings preferred.
- Excellent communication (written and verbal) and critical thinking skills required.
