Job Summary

We are looking for a Part‑Time Social Worker (24 hrs per week) to work from 9:00 am to 3:30 pm at Woodbine Rehabilitation and Healthcare Center, located in Alexandria, Virginia.

• Provide social work services to patients, including counseling, case management, and discharge planning.
• Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary care team.
• Facilitate patient and family education, communication, and support to ensure a smooth transition to home or next‑level care.
• Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
• Develop and maintain relationships with patients, families, and community resources to enhance patient outcomes.
• Participate in quality improvement initiatives to continually improve patient care and services.

• Minimum 1 year of experience in social work or a related field.
• Bachelor’s degree in Social Work or completion of a social work program with field placement.

• Familiarity with social work theories, principles, and practices.
