Overview
The Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The RN Case Manager manages symptoms of the patient's disease and re-evaluates effectiveness of interventions and adjusts the care plan as needed. The RN Case Manager plans and manages the nursing component of the hospice caseload.

ResponsibilitiesThe Registered Nurse (RN) Hospice Case Manager provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregivers. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs. The RN Case Manager is responsible for supervising Licensed Practical Nurses, Certified Nursing Assistants, and Patient Care Volunteers. May be asked to rotate to other regions/departments.
QualificationsExperience Requirements Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly desired.
Education Requirements Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.
Required Certificates and/or Licenses Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus.
Driver's License Must have a driver's license in good standing and access to a vehicle during working hours.
