Pay: Between $34 and $38 per hour, depending on experience
Schedule: 7 am–3 am Monday, Tuesday, Wednesday each week
Location: Based in Falls Church, VA, with visits to the surrounding community and our three campuses
About the PositionAs the Home Care LPN, you will support the Home Care Clinical team by conducting in‑home functional assessments for Goodwin Living At Home members, assisting with new resident assessments under the supervision of the Clinical Nurse Manager, performing supervisory visits for residents receiving home care services, and submitting long‑term care insurance documentation for reimbursement. This position requires travel within the community and service to residents on our three campuses.
KeyDuties
- Conduct in‑home supervisory visits every 90 days for residents receiving services, in accordance with state regulations.
- Perform functional assessments for Goodwin Living At Home (GLAH) members quarterly, and assess eligibility for those requesting services.
- Collaborate with residents, families, and team members to develop Home Care Agreements on a PRN basis.
- Conduct medication management visits to residents’ homes weekly and PRN to refill pill boxes per medication orders.
- Submit long‑term care (LTC) insurance documentation monthly and as needed to support reimbursement.
- Develop and coordinate quality service to ensure resident, client, and employee satisfaction.
- Help others understand the value of treating everyone with dignity and respect.
- A valid driver’s license and up‑to‑date car insurance.
- A Licensed Practical Nurse currently licensed in the Commonwealth of Virginia or a compact state.
- Active CPR certification.
- At least one year of experience in home care with direct health care service delivery.
- Working knowledge of state, federal, and local regulations, rules, and guidelines.
- Ability to effectively communicate in understandable English.
- Sound computer knowledge and skills, including an aptitude for using health information technology.
