Role OverviewThe Admission Nurse is responsible for the care of patients assigned by the Director of Hospice Admissions. The RN, under the physician’s nursing order, will conduct admission assessments, develop plans of care, and provide direct nursing care in a Hospice setting.
Key Responsibilities- Present hospice services and philosophy of care to patients and families.
- Conduct in‑person assessments to determine hospice eligibility, obtain physician certification, and obtain consent for service.
- Complete admissions process, initiate plan of care, and ensure immediate care needs are addressed.
- Communicate goals of care and initial plan of care to the interdisciplinary team.
- Provide direct nursing care under the guidelines of a physician’s order.
- Perform admission/start of care (SOC), recertification, evaluations, discharges, and documentation in a timely manner.
- Develop a patient‑specific plan of care and document it in the clinical record per agency policy.
- Collaborate with team members to maintain patient’s case throughout the care continuum.
- Document accurate treatment notes and other necessary documentation as directed by the physician.
- Prepare patient discharge planning and communicate it to the patient, team members, and physician.
- Document all communication with other personnel to contribute to patient’s case conference notes.
- Maintain confidentiality of patient information and employee personal information.
- Implement infection control and safety measures as appropriate.
- Promote education for patients, family members, and caregivers.
- Attend mandatory in‑services and maintain 100% compliance with mandatory data‑based training.
- Graduate of an accredited school of nursing.
- Licensed as a Registered Nurse in the Commonwealth of Virginia.
- At least one year of acute care experience; hospice experience preferred.
- Current CPR card.
- Effective verbal and written communication in English.
- Knowledge of Home Health and Hospice care preferred.
- Proficiency in HIS documentation and SOC.
- Independent decision‑making and work ability throughout the workday.
- Driver’s license, proof of automobile insurance, and reliable transportation.
- Compassionate and tactful approach to patients, family members, and caregivers.
- Willingness to participate in a teamwork approach with all disciplines involved in patient care.
- Financial assistance with U.S. Citizenship application or DACA renewal.
- Tutoring for ESL, Citizenship Test & GED.
