The LPN provides practical nursing services under the supervision of a Registered Nurse.

• Accept assignments of clients from the R.N. Case Manager.
• Provide nursing care through the utilization of the home assessment, nursing diagnoses, and physician‑approved care plans.
• Perform treatments ordered by the physician and delegated by the R.N. Case Manager.
• Observe signs and symptoms and report them to the R.N. Case Manager.
• Note changes in physical or emotional status of clients.
• Promote client and caregiver independence in self‑care.
• Utilize electronic documentation of client care; comply with Agency policies regarding timeliness in submission of documentation and synchronization of data.
• Follow Agency policies regarding universal precautions and safety guidelines.
• Fulfill other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

• Holds a current Licensed Practical Nurse license in Virginia.
• Has a minimum of one year of nursing experience.
• Has current CPR certification.
• Has excellent communication, organizational, and human relations skills.
• Can function with minimal supervision.
• Can manage multiple projects and changing priorities.
• Can employ critical thinking skills.
