Shelter House was established in 1981 as a grassroots response to the homelessness crisis in Fairfax County. Since its inception, the organization has grown and adapted to meet evolving community needs in partnership with government agencies, private partners, and community members. Today, Shelter House is a respected nonprofit with more than 100 employees dedicated to preventing and ending homelessness and domestic violence.

The Role
The Mondloch House and Adult Protective Services (APS) Case Manager provides case management services to single adults residing in the Mondloch House emergency shelter, as well as APS clients referred for support. Clients may face complex challenges such as mental health diagnoses, substance use, physical or developmental disabilities, limited income, and chronic homelessness. The Case Manager supports clients in obtaining and maintaining housing stability through best practices including trauma informed care, housing first, and harm reduction.
This role collaborates closely with the Community Services Board, Department of Family Services, and other partner agencies to ensure comprehensive service delivery.
- Use a strengths‑based approach to assess client strengths, physical and mental health, social needs, education, life skills, and employment readiness.
- Partner with clients to identify supports needed to maintain stability and achieve short and long term goals.
- Develop individualized service plans tailored to client needs.
- Support clients in engaging in social and community activities and help remove barriers to participation.
- Assist clients in obtaining appropriate services, including employment, housing, financial assistance, mental health support, and SSI applications.
- Provide crisis intervention and implement emergency procedures when necessary.
- Assist clients with budgeting and money management, ensuring rent and utilities are paid on time.
- Maintain complete and accurate client records in accordance with agency and best practice standards.
- Document all client interactions in client files and HMIS and complete annual reassessments and quarterly reviews.
- Participate in staff meetings and trainings as required.
- Ensure compliance with all agency policies.
- Collaborate as part of a multidisciplinary team within Shelter House and with partner agencies.
- Share relevant client information with appropriate staff in a timely manner.
- Use Microsoft Office Suite applications as part of daily work.
- Safely operate Shelter House vehicles to transport clients when needed.

- Bachelor’s degree in human services or related field, or equivalent experience.
- Two or more years of case management, direct services, or related experience.
- Strong written and verbal communication skills.
- Ability to prioritize competing needs and exercise sound judgment.
- Ability to work effectively despite frequent interruptions.
- Creative, resourceful approach to problem solving.
- Strong organizational and time management skills.
- Valid Driver’s License and reliable transportation.
- Ability to drive a 15‑passenger van.
- Willingness to work evenings or weekends as needed.
- Fluency in a second language such as Spanish, Farsi, Arabic, or Amharic.
- Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders.
- Experience using HMIS.
- Annual TB test.
- Ability to sit or stand for extended periods.
- Ability to lift 10 to 20 pounds.
