Overview

COMMUNITY SUPPORT SPECIALIST — PART-TIME UP TO 6 HOURS PER WEEK with potential to increase hours due to the Veteran needs. Simple Steps, LLC currently has a Community Support Specialist position open in the Quincy, FL area.

Qualifications
  • High School Diploma
  • Experience with physical and cognitive impairments/disabilities
  • Experience with Traumatic Brain Injury
  • Experience with Post-Traumatic Stress Disorder
  • Veteran/Military Experience
  • Recreation Therapy Aides, Physical Therapy Aides, Occupational Therapy Aides
Responsibilities
  • Deliver high-quality home and community services to Veterans with neurological disorders, physical disabilities, and traumatic brain injuries
  • Support and carry out quality client care plans; ensure resources are engaged to meet client needs
  • Orient/educate clients and families about health care plan services and procedures
  • Support accurate and complete documentation; provide timely data for generating invoices
  • Collaborate with Case Managers and family members to provide dynamic social, recreational, wellness, educational, and life skills activities in home and community settings
  • Report to assigned Care Coordinator
Essential Duties and Responsibilities
  • Must be able to travel to current or prospective clients’ homes
  • Must be able and willing to transport client(s) during visits
  • Must have reliable transportation, a valid driver’s license, and agency-approved auto liability insurance (100k/300k Bodily Injury)
  • Communicate with health care professionals, clients, families, and the public through verbal and written communication
  • Have access to a private personal laptop or desktop computer for HIPAA compliance
  • Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader
  • Implement patient care plans through home and community goal-related activities
  • Able to work independently and manage one’s own time and client time
