Kenniscentrum Sociaal Domein zoekt een Consulent Participatie die inwoners met een afstand tot de arbeidsmarkt begeleidt naar werk en participatie. Je beheert zelfstandig een caseload en voert brede intakes uit met inwoners.
Je beoordeelt wat iemand nodig heeft volgens de Participatiewet, stelt een plan van aanpak op en zet re-integratie- en participatietrajecten in. Ook voer je coachings- en voortgangsgesprekken en werk je samen met partners binnen het Sociaal Domein.
