Senior Practitioner - Children with Disabilities Team

Clackmannanshire Children with Disabilities Team is seeking an experienced, enthusiastic, and highly motivated Senior Practitioner to join our small, dedicated, and dynamic team.

This is an exciting opportunity to play a key leadership role within a service committed to achieving the very best outcomes for children and young people with disabilities and their families. We are a small hardworking and supportive team that prides itself on building meaningful relationships, working creatively to overcome challenges, and ensuring children and young people receive the right support at the right time.

As a Senior Practitioner, you will work alongside passionate and skilled colleagues who are committed to promoting inclusion, safeguarding children, supporting families, and enabling children and young people with disabilities to reach their full potential. You will provide professional support, mentoring, and guidance while managing a complex caseload and contributing to the ongoing development of the service.

The team works with children and young people with a wide range of complex needs and disabilities, supporting families through assessment, planning, transition to adulthood, Self-directed Support (SDS), short breaks, complex care packages, and multi-agency interventions. We are committed to delivering relationship-based, trauma-informed, and strengths-focused practice that keeps children, young people, and their families at the heart of everything we do.

Key Responsibilities

As a Senior Practitioner within the Children with Disabilities Team, you will:

  • Manage and oversee a complex caseload involving children and young people with disabilities and their families.
  • Provide professional support, supervision, mentoring, and guidance to Resource Worker

  • Lead on the development of effective transition planning for young people moving into adult services.

  • Complete high-quality assessments, care planning, reviews, and risk management processes, ensuring interventions are child-centred and outcome-focused.
  • Undertake and contribute to Pre-Birth Assessments, supporting timely decision-making and effective planning for vulnerable children and families.
  • Lead and contribute to child protection investigations, Child Protection Planning Meetings, Care Experienced Children processes, and complex multi-agency interventions.

  • Ensure practice is trauma-informed, strengths-based, relationship-focused, and aligned with GIRFEC principles.
  • Support children and young people through key life stages, including transition planning to adulthood, Self-directed Support (SDS) processes, short breaks, and complex care package arrangements.
  • Work collaboratively with partner agencies, including health, education, police, adult services, housing, and third-sector organisations, including attendance at Initial Referral Discussions (IRDs) and other statutory meetings.
  • Provide professional leadership and expertise in relation to disability, safeguarding, risk assessment, and legislative responsibilities.
  • Support the effective allocation and oversight of work within the team, contributing to the delivery of high-quality services and positive outcomes for children and families.
  • Contribute to service improvement, quality assurance, performance management, and the implementation of best practice across the service.
  • Promote inclusion, participation, and the rights of children and young people with disabilities, ensuring their voices influence assessment, planning, and decision-making.

In return, we offer more than just a job. We offer the opportunity to be part of a small team where your contribution is valued, your ideas are welcomed, and your professional development is actively supported.

Join us and help shape positive futures for children and young people with disabilities and their families across Clackmannanshire.
