MDS Coordinator / Registered Nurse

Michigan Masonic Home is seeking a dedicated, compassionate, and skilled MDS Coordinator/Registered Nurse who is passionate about serving others. In this role, the MDS Coordinator/RN Charge Nurse will oversee the Short-Term Rehab Unit, ensuring smooth day‑to‑day operations and providing exceptional supervision.

• Conduct and coordinate the completion of required Resident Assessment Instrument (RAI) and implement ongoing evaluation of each resident’s comprehensive plan of care.
• Accurately complete all MDS assessments and any supporting documentation.
• Audit medical records for supporting documentation for all items coded on the MDS and provide education to department leaders, physicians, and other staff as needed.
• Collaborate with health plans, physicians, and the interdisciplinary team to assure insurance approvals and/or that residents meet eligibility and coverage criteria.
• Schedule the Assessment Reference Dates (ARDs) for payment assessments to capture each resident’s clinical characteristics for payment classification purposes.
• Provide a schedule of ARDs and assessment types weekly to facilitate timely completion of MDS sections and CAAs by each discipline.
• Transmit assessments in accordance with current regulations, as frequently (daily) as necessary to obtain timely validation of MDS acceptance into the Quality Improvement and Evaluation System (QIES) Assessment Submission and Processing (ASAP).
• Facilitate the completion and updating of resident care plans with the IDT to reflect each resident’s current needs.
• Communicate with the IDT to identify residents in need of a Significant Change in Status assessment based on criteria in the current RAI manual.
• Participate in all resource utilization and/or triple‑check meetings prior to billing of claims associated with payment assessments.
• Assist with admission, discharge, or care of residents as needed.

• Current, unencumbered, active license to practice as a registered nurse in this state.
• At least 3 years’ experience as a registered nurse, including 1 year of supervisory experience. Experience with MDS completion preferred (3–5 years).
• Resident Assessment Coordinator (RAC) training completed within six months of hire; RAC Certification preferred.
