Doenersdreef Zorg zoekt een (gezins) begeleider voor jongeren en jongvolwassenen met een (licht) verstandelijke beperking in Almere. Je biedt dagelijkse ondersteuning en werkt flexibel binnen team Begeleid Zelfstandig Wonen (BZW).
Je sluit aan bij pedagogische hulpvragen, coördineert zorgplannen en betrekt het netwerk rondom de cliënt. Je bent bereid tot onregelmatige diensten, inclusief dag-, avond- en weekenddiensten.
