Summary
Responsible for performing basic, personal care services in the home when an individual or family cannot perform these tasks. The aide will care for patients of all ages, with all types of diagnoses and in many different phases of illness.
Note:
Certified Home Health Aides(CHHA) with additional home care training may perform additional responsibilities as directed by the care plan or as directed by the Registered Nurse (RN) or Medical Licensed Pratical Nurse (MLPN).

Responsibilities
(10%)
* Assesses care necessary for client health needs and activities of daily living as directed by the RN case manager and recognizing the specific individual needs of the adolescent, adult and geriatric population.
(10%)
* Plans and prioritizes client care functions to meet clients' health care needs.
(50%)
* Implements actions that compliment and support nursing care.
(10%)
* Evaluates the effectiveness of care provided to ensure quality care and timely completion of client-related activities.
(20%)
* Self-Management:
Holds self accountable for delivery of responsible nursing care, demonstrates effective communication skills and interpersonal relationships.
Assumes responsibility for own continued development and assists with the development of other staff members.

Operates equipment as outlined on orientation checklists, policy and procedures.
Participates in equipment in-services and competencies as they relate to role expectations.
Performs job skills outlined on orientation checklist as they relate to role expectations.
Participates in Continuous Quality Improvement as required by the Health System and the job description.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL
WORKING CONDITIONS:
Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
Overall vision and hearing is necessary with or without assisted device(s).
Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.
