Kalamazoo Long Term Care Pharmacy is looking for a part-time Consultant Pharmacist to work on site at the PACE center we support in Alpena. PACE (Program of All-Inclusive Care for the Elderly) helps older adults stay safely in their homes and communities. Medication management is central to that work.

In this role you’ll be the on-site pharmacy expert and a core member of the center’s care team. You’ll have the time and the relationships to make a real difference in participants’ therapy. What you’ll do- Perform comprehensive medication reviews and reconciliations for PACE participants, including at enrollment, at care transitions and after hospital stays
- Join care planning meetings and reassessments with the interdisciplinary team (IDT)
- Work with physicians, nurse practitioners and nursing staff to resolve medication issues and support evidence-based prescribing
- Identify polypharmacy, high-risk medications and chances to deprescribe
- Act as the liaison between the center and our pharmacy, resolving refill, delivery and MAR issues in real time
- Oversee on-site medication storage, inventory and emergency supplies
- Teach center staff, participants and caregivers about medications, adherence and ordering processes
- Support formulary management, quality improvement and compliance with CMS PACE and Michigan Board of Pharmacy rules
- PharmD from an ACPE-accredited program
- Active, unrestricted Michigan pharmacist license, or eligibility to get one before you start
- Knowledge of CMS PACE and long-term care pharmacy regulations
- Strong communication skills, problem-solving ability and attention to detail
- The confidence to work independently on site while collaborating with an interdisciplinary team
- Long-term care, PACE or geriatric pharmacy experience
- Board Certified Geriatric Pharmacist (BCGP)
- Experience with Framework or Medi Procity
- Meaningful clinical work with a population that benefits greatly from pharmacist involvement
- A consistent part-time schedule of one day per week
- Competitive pay of $61–$67 per hour
