Advance Practice Nurse-Primary Care Regional Medical Director

Summary

The Regional Medical Director APN is a licensed independent practitioner with advanced education and national certification. This position will assist the Regional Medical Director for Primary Care in providing leadership and direction for Primary Care Providers - Clinical leaders and administrative support

• Assist Nurse leaders - AON's and interdisciplinary clinicians in carrying out the functions and activities expected of them
• Contributes ideas and recommendations for the establishment of standards of care - policies - and objectives doe the enhancement of clinical practice
• Leads and collaborates in policy-making activities related to customer service and the overall functioning of administrative and clinical programs
• Collaborates with other health care staff to establish and maintain programs that cross services and/or disciplines that influence organizational mission and health care
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacological and non-pharmacologic interventions
• Initiates referrals and consultations on specialty services
• Provides education to patients and their families Assesses and adjust the plan of care as needed to achieve optimal patient outcomes
