Licensed Practical Nurse Home Health | Marlborough, MA

Sunbelt Staffing is currently seeking a dedicated and experienced Home Health Licensed Practical Nurse (LPN) for a rewarding 13-week contract assignment in Marlborough, MA. The ideal candidate must have at least 1 year of recent home health experience and possess proven proficiency with Home Care Home Base (HCHB) within the last year. A current American Heart Association BLS certification is also required.

  Monday – Friday, 8:00 AM – 5:00 PM

• On-Call:
  Discussed in an interview

• Active LPN license in the state of assignment or compact license

• Minimum 1 year of recent home health experience

• Demonstrated HCHB (Home Care Home Base) experience within the last 12 months – this is a strict requirement

• American Heart Association BLS certification

• Conduct patient assessments and reassessments in the home setting

• Collaborate with physicians and interdisciplinary teams to implement individualized care plans

• Administer medications and perform skilled nursing procedures (e.g., wound care, IVs)

• Document care using HCHB and maintain timely, accurate clinical records

• Educate patients and caregivers on health management and treatment plans

• Participate in case conferences and contribute to ongoing care coordination

• Respond to emergencies and initiate appropriate interventions

• Comply with all agency policies, procedures, and applicable regulations
