Sunbelt Staffing is currently seeking a dedicated and experienced Home Health Licensed Practical Nurse (LPN) for a rewarding 13-week contract assignment in Butler, GA. The ideal candidate must have at least 1 year of recent home health experience and possess proven proficiency with Home Care Home Base (HCHB) within the last year. A current American Heart Association BLS certification is also required.

Monday – Friday, 8:00 AM – 5:00 PM - On-Call:

- Active LPN license in the state of assignment or compact license
- Minimum 1 year of recent home health experience
- Demonstrated HCHB (Home Care Home Base) experience within the last 12 months – this is a strict requirement
- American Heart Association BLS certification
- Conduct patient assessments and reassessments in the home setting
- Collaborate with physicians and interdisciplinary teams to implement individualized care plans
- Administer medications and perform skilled nursing procedures (e.g., wound care, IVs)
- Document care using HCHB and maintain timely, accurate clinical records
- Educate patients and caregivers on health management and treatment plans
- Participate in case conferences and contribute to ongoing care coordination
- Respond to emergencies and initiate appropriate interventions
- Comply with all agency policies, procedures, and applicable regulations
- Maintain professional development and stay updated on best practices
