Position Summary

The Physical Therapist (PT) provides medically prescribed, goal-oriented therapy in accordance with the Physician’s Plan of Care, company policies, and professional standards. This position requires collaboration with the interdisciplinary team to deliver high-quality, patient-centered care in the home setting.

Key Responsibilities
  • Develop and implement individualized physical therapy plans of care.
  • Perform in-home visits to evaluate, treat, and reassess patient needs.
  • Communicate regularly with physicians, care coordinators, and interdisciplinary team members.
  • Educate patients and families on home exercises, safety, and wellness strategies.
  • Participate in interdisciplinary team meetings to review and coordinate care plans.
  • Maintain accurate and timely documentation within the EMR system.
  • Comply with all agency policies, ACHC standards, and federal/state regulations.
Qualifications
  • Current Florida PT license in good standing.
  • Specialized Training And Experience In Pelvic Floor Therapy Required.
  • Knowledge of physical therapy principles, concepts, and methodologies.
  • Excellent communication, critical thinking, and problem-solving skills.
  • Ability to work effectively in a collaborative healthcare environment.
Required Credentials
  • Current CPR certification
  • Level 2 Background Screening
  • Recent negative TB test or chest X-ray
