Position SummaryThe Physical Therapist (PT) provides medically prescribed, goal-oriented therapy in accordance with the Physician’s Plan of Care, company policies, and professional standards. This position requires collaboration with the interdisciplinary team to deliver high-quality, patient-centered care in the home setting.
Key Responsibilities- Develop and implement individualized physical therapy plans of care.
- Perform in-home visits to evaluate, treat, and reassess patient needs.
- Communicate regularly with physicians, care coordinators, and interdisciplinary team members.
- Educate patients and families on home exercises, safety, and wellness strategies.
- Participate in interdisciplinary team meetings to review and coordinate care plans.
- Maintain accurate and timely documentation within the EMR system.
- Comply with all agency policies, ACHC standards, and federal/state regulations.
- Current Florida PT license in good standing.
- Specialized Training And Experience In Pelvic Floor Therapy Required.
- Knowledge of physical therapy principles, concepts, and methodologies.
- Excellent communication, critical thinking, and problem-solving skills.
- Ability to work effectively in a collaborative healthcare environment.
- Current CPR certification
- Level 2 Background Screening
- Recent negative TB test or chest X-ray
