CCBHC Community Outreach Specialist

Centerstone is a leading nonprofit behavioral health system dedicated to delivering care that changes people’s lives.

As a Community Outreach Specialist, you will develop and implement outreach strategies that increase awareness of Centerstone services, build and maintain relationships with community members, attend events, and provide comprehensive outreach services including education, linkages to services, and support continuity of care.

• Know Centerstone programs and services, eligibility requirements, and how to start services.
• Coordinate with team members, families, and community partners to ensure continuity of care for clients.
• Increase contacts with community organizations to generate client referrals and maintain relationships to encourage use of Centerstone’s programs.
• Complete enrollment and assessment for eligible participants, maintaining accurate records and documentation.
• Conduct ongoing assessment and evaluation of community needs and adapt outreach strategies accordingly.
• Implement strategies to inform the public about the mission of the organization and develop CCBHC clinics.
• Work independently and resourcefully, seeking out additional duties as needed.

• Community engagement, relationship-building, and networking skills.
• Customer service techniques.
• Responsible delegation, time management, and prioritization.
• Proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and Electronic Health Records.
• Knowledge of clinical best practices and confidentiality guidelines.

• Master’s degree in Human Services or related field. Licensure eligibility required.

• Minimum one year providing behavioral health services.
• Preferred one year of experience in community outreach or a similar field.

• Pay range: $43,900.00 – $61,370.00
