We serve over 100 residents with dozens of locations throughout the Illinois‑Metro East area. With a team of highly trained and dedicated care professionals, we offer 24/7 services and choice of the model of home (Community Integrated Living Arrangement or Intermediate Care Facility/Developmental Disability) that best meets a person's preference.

    Provides on‑call services to assigned group homes from 3pm-7am Monday thru Friday and scheduled weekends/evenings. Must respond within 15 minutes.

    : 25‑40% between IL sites to travel to meetings, trainings, or to transport residents.
Responsibilities
  • Promote a culture of person‑centered services that advances the Company's mission to provide quality care and services in a community‑based setting to individuals with disabilities. Ensuring a high rate of resident satisfaction, ensure the health and safety needs of all residents are met by providing nursing services to residents.
  • Recruit, screen, and interview prospective residents as requested. Attend all resident PP/ITP, IHP staffing meetings in assigned homes or send medical summary prior to the meeting.
  • Plan, organize, and supervise the delivery of care to residential residents. Complete annual nursing assessment, quarterly nursing assessment, TD, self‑medication assessment and monthly observations, admission, NSP'S, TB signs and symptoms check‑list, influenza immunization, tetanus, pneumococcal, and hepatitis series as ordered by physician. Visit assigned Residents no less than bi‑weekly to evaluate their current condition and assist in healthcare planning. Complete reports and communicate key information to the VP of Residential Options in a timely, accurate, and comprehensive manner.

    Ensure that each facility meets state, federal, CARF (Commission, on Accreditation of Rehabilitation Facilities) and any governing agency's regulatory guidelines.
  • Maintain records of assigned Residents for medications, charts, vaccinations, and annual assessments accurately and timely. Review all Physician Order Sheets and Medication Administration Records prior to being placed in the Resident's chart in a timely manner at least monthly. Review and sign incident reports with follow‑up as needed. Review all medication errors reports, complete medication error form and forward to Public Health, DHS and OIG (Office of Inspector General) according to regulations.

    Review all labs and forward to the prescribing physician for review. Communicate effectively with physicians and service providers regarding Resident care. Order medical supplies as needed. Assist with and oversee the training of direct care staff on Basic Health and Safety, Medication Administration and other health related topics as required. Provide immunizations and TB screenings to employees and maintain accurate records.
  • Answer calls in a timely manner and provide on‑site services as required within 24 hours. Follows through with reported problems and communicates the mto supervisor/primary care nurse. Check nurses' mailbox at group homes weekly.
  • Lead team and place people in positions to succeed. Hire, train, and empower top‑notch leaders to effectively oversee their areas of responsibility. Encourage effective outcomes and accountability. Plan, develop, organize, implement, direct, and evaluate assigned sites' activities and performance. Communicate job expectations. Plan, monitor, and appraise job results; evaluate team for continual improvement, efficiency and effectiveness. Initiate, coordinate, and enforce systems, policies, and procedures.
