You will coordinate with a multidisciplinary team to deliver patient-centered care at the patient’s home, using strong clinical judgment and OASIS knowledge.
Requires RN license in IL or MI, BLS, and a valid driver's license. Familiarity with EMR and Microsoft tools is beneficial, with BSN preferred.
