Clinical Nurse Liaison
Option Care Health is a national leader in home and alternate site infusion services. We seek a licensed RN to provide clinical education, training, and direct patient care as part of our care delivery model.
Responsibilities- Evaluate, educate, and train patients, caregivers, and facility staff about how OCH services & products will be facilitated in an alternative site, in‑home, or virtually.
- Conduct patient assessments and evaluate patient viability to join the OCH care delivery model.
- Partner with the OCH sales team to understand clinical transition educational needs and develop improvements to increase patient transition volume.
- Maintain confidentiality of patient and proprietary information in accordance with HIPAA and legal guidelines.
- Provide proper documentation of education using OCH applications and technology.
- Coordinate patient transition from hospital to home or alternate care settings, ensuring timely discharge, therapy education, and continuity of care.
- Provide in‑person patient and caregiver education, gather pre‑admission and insurance information, and facilitate the initiation of services to support safe and effective home care.
- Initiate and implement intermediate nursing plans of care for infusion treatment, administer medications, and educate patients and families.
- Assess patient needs and physical status at each visit; re‑evaluate progress, obtain physician orders, and communicate significant changes.
- Train and mentor lower‑level infusion nurses.
- Communicate timely with the clinical team, medical providers, patients, and families to facilitate continuity of care.
- Prepare clinical documentation in real time and submit to the nursing department in compliance with billing timelines.
- Coordinate discharge planning and prepare discharge summaries with patient instructions.
- Demonstrate compliance with agency operations, policies, procedures, and all regulations and standards.
- Use computers and automation technologies to validate care plans and manage visit schedules.
- Respond promptly to patient requests and initiate emergency procedures as needed.
- Practice ethical and professional conduct and adhere to established programs and policies.
- Attend required training modules, in‑services, and continuing education to maintain competency and licensure.
- Provide oversight and coordination of paraprofessionals in the home setting; act as supervisor in the absence of a supervisor or nurse manager.
- Participate in multidisciplinary team conferences and mentor other nurses as needed.
- Serve on nursing department on‑call responsibilities.
- Perform other related duties as directed by supervisor.
Education and Experience Requirements
- Active and unrestricted Registered Nurse (RN) license required.
- Minimum of 2 years of nursing experience.
- Current CPR certification required.
- Demonstrated competency in patient care standards for safe delivery of infusion services; advanced certification and training as applicable.
- Experience establishing and maintaining relationships with individuals at all levels of the organization and with vendors.
- Experience applying knowledge of standard practices for all services offered, including ACHC and URAC standards.
- Experience providing customer service to internal and external customers, meeting quality standards and evaluating satisfaction.
- Basic PC skills: competency in internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
- Access to a reliable means of transportation, including a valid driver’s license and proof of insurance if using a personal vehicle.
- Willingness to obtain nursing licensure in additional states if required.
- Ability to plan, organize, and make presentations.
- Hands‑on experience with home or alternate site infusion or discharge planning.
- Bachelor of Science in Nursing preferred.
- Experience growing service provider partnerships.
- 100% local travel to and from patients’ homes, alternate infusion sites, partner facilities, community hospitals, and medical practice offices for sales, referrals, and training.
