- Conducts comprehensive assessments to establish individuals’ strengths, limitations and treatment plans to improve their well‑being, health, and quality of life.
- Develop the appropriate therapy treatment intervention based on the needs of the individual, in addition report progress of long‑ and short‑term treatment goals.
- Introduce new skills and approaches for individuals to achieve in daily life.
- Evaluate individuals’ living space and/or other environment to ensure treatment plans meet safety protocol as well as consistent with the individuals’ resources.
- Provides support, counsel, advice, training and education for individuals, family, caregivers and other professionals on the importance of being consistent with therapy treatment plans.
- • Efficiently assembles extensive documentation on individuals’ progress, care services and therapy adjustments, when necessary, that aligns with standard medical documentation and facility protocol/procedures documentation.
- Collaboratively work with other healthcare professionals to provide comprehensive care.
- Must display excellent active listening skills, critical thinking skills, verbal/written communication skills, time management skills.
- Must display compassion, integrity and respect to all individuals.
- Supporting additional responsibilities and/or tasks as needed for the department.
- Physically, be able to lift, transfer and assist patients while providing therapy interventions during physical activity.
- Must be able to lift and carry items up to 25lbs, including assisting individuals when needed.
- Ability to visually be able to perform job duties, including examining individuals and reading information from printed sources and computer screens.
- Education:
Must be a graduate of an accredited Occupational Therapy program. - Licenses/Certifications:
Must hold an active and in good standing Wisconsin Occupational Therapist License and CPR Certification. - Follows/adheres to organization, federal, and state regulations and guidelines, including Resident/Client rights.
- Maintains neat, well‑groomed appearance, adhering to dress code of organization/program.
