As an independent lease holder in Walmart stores, you will lead patient care, manage your own practice schedule, and maintain autonomy over fee structures and operations.

• Launch and operate your eye care services within Walmart or Sam's Club stores.
• Establish your days of practice and hours of operation.
• Control your patient schedule.
• Set your fee schedule and retain 100% of exam/professional fees.
• Maintain high standards of patient care and comply with all applicable regulations.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school (optional).
• Valid state ophthalmology license (optional).
• General Liability Certificate/Professional Liability Insurance Certificate.

• Advanced Equipment/instrumentation.
• Administrative and/or Opt-Tech support services if state allows.

ME/VA license agreements are with Gumberg Asset Management Company.
