- The Memory Care Coordinator oversees the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services, including engagement and supports the HWD in all health services.
- Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community.
- Ensures residents satisfaction and achievement of their realistic goals. Motivates residents to function at highest level of independence.
