Job Overview

As the Director of Nursing, you will lead our clinical team and ensure the highest standards of patient care. Your key responsibilities include:

Provide Leadership & Oversight
  • Guide and mentor nursing staff and home health aides, fostering a culture of excellence and collaboration to ensure quality and continuity of care.
Ensure Clinical Competency
  • Conduct regular staff competency assessments and implement training initiatives to maintain compliance and professional growth.
Champion Our Mission & Standards
  • Promote the organization’s mission, values, and policies while ensuring adherence to regulatory requirements and best practices.
Manage Patient Care
  • Perform ongoing assessments of patients in their homes, develop individualized care plans, and evaluate outcomes to optimize health and well‑being.
Coordinate Care Across Teams
  • Collaborate with physicians and interdisciplinary teams to align services with each patient’s plan of care.
Oversee Staffing & Scheduling
  • Assign patient care responsibilities, manage staff schedules, and ensure adequate coverage for all shifts.
Drive Quality & Compliance
  • Monitor compliance with state and federal regulations, participate in quality assurance and performance improvement (QA/PI) activities, and implement corrective actions as needed.
On‑Call Leadership
  • Provide on‑call support to address urgent clinical needs and maintain continuity of care.
Job Requirements
  • Active Registered Nurse (RN) license in the state of practice (BSN preferred)
  • 5+ years of nursing experience; home health or transitional care experience preferred
  • Management experience preferred
  • Strong communication and interpersonal skills, with a focus on patient‑centered care
  • Knowledge of DOL/EEOICPA guidelines and home health regulations is a plus
  • Proficient in EMR systems; experience with IGEA preferred

$95,000 - $110,000 a year

Offers are contingent upon passing a background check.
