Je werkt in een multidisciplinair team en biedt ondersteuning in de thuissituatie. De rol vereist een hbo-opleiding en ervaring met ambulante zorg. Als je twijfelt, kun je een dag meelopen voor de sollicitatie. Salaris volgens CAO GGZ, van € 3.456 tot € 4.579 bruto per maand.
