Humanitas DMH zoekt een betrokken begeleider voor het Homerunhuis in Amersfoort. Je werkt voor 28-32 uur per week en ondersteunt jongeren met LVB en multi-problematiek bij het bereiken van zelfstandigheid binnen een vier-fasen traject.
Je bent het dagelijkse aanspreekpunt, motiveert, en onderhoudt relaties met het netwerk. Je hebt een mbo-4 zorgopleiding en coachende vaardigheden om samen met het team doelen te realiseren.
