Humanitas DMH zoekt een persoonlijk begeleider in Amersfoort voor 28–32 uur per week. Je stelt samen met jongeren een trajectplan op en werkt hierbij nauw samen met de trajectcoördinator en gedragsdeskundige. Je biedt begeleiding en ondersteuning en bent het aanspreekpunt.
Je hebt ervaring met de doelgroep, een zorggerelateerd HBO-diploma en een coachende, open houding. Ook het afgeven en ontvangen van feedback is onderdeel van de rol.
