De Ontdekking biedt in Amersfoort 24-uursbegeleiding aan 31 bewoners met LVB+ in een oud schoolgebouw. Als Begeleider LVB+ werk je met een hecht team aan dagelijkse ondersteuning en persoonlijke begeleiding van bewoners.
Je taken omvatten het opstellen en uitvoeren van begeleidingsplannen, registreren van dossiers, en afstemmen met doktersbezoeken en activiteiten. Je zorgt voor rust en structuur, past presentie en motiverende gespreksvoering toe, en onderhoudt contact met bewoners en familie.
