Job SummaryWe are looking for an experienced Care Co‑ordinator to develop and maintain a customised Community Care/Support Service throughout the Buckinghamshire region. The role involves organising individualised care and resources for clients in their homes and in the community, ensuring compliance with policies and registrational requirements, and maintaining composure under a fast‑paced environment.
Primary Responsibilities- Ensure all applicable policies, procedures and regulations are followed when providing care to clients.
- Oversee carer roster allocations to guarantee that every client’s needs are met.
- Assess prospective clients to determine unique care requirements.
- When necessary, revise current care plans and re‑evaluate existing clients.
- Communicate with families, medical professionals and other resources as needed.
- Deliver top‑notch customer service to internal and external stakeholders.
- Conduct prompt spot checks, supervisions and care worker evaluations.
- Serve on a rotating team to provide on‑call coverage for after‑hours services.
- Ensure clear and effective communication among management and staff.
- Maintain service timeliness and continuity of care.
- Set reporting channels and offer solution‑focused feedback.
- Use processes and systems efficiently.
- Advance inclusion, diversity and equality initiatives.
- Effectively manage employee hours and rota schedules, ensuring proper breaks, rest periods, travel time and maximum/minimum weekly hours, with accurate electronic timesheets.
- Adapt to fulfil home care service’s operational requirements and provide community care when needed.
- Transfer employees between service users when necessary.
Permanent, Full‑Time
Pay£24,500.00 - £26,500.00 per year
