Kenniscentrum Sociaal Domein in Amstelveen zoekt een Wmo Consulent met een HBO-opleiding en aantoonbare Wmo-ervaring. Je indiceert voorzieningen en biedt inwoners begeleiding en ondersteuning, met als doel zelfredzaamheid en participatie te vergroten.
Je behandelt (complexe) Wmo-aanvragen, verricht huisbezoeken en onderzoekt welke oplossing het beste past. Je werkt integraal samen met collega’s, levert rapportages aan en bewaakt de kwaliteit.
