Je biedt structuur, begeleiding en continuïteit van zorg aan cliënten met een ernstig psychiatrische aandoening (EPA). Je werkt samen met een multidisciplinair team, voert intakegesprekken, huisbezoeken en behandelingen uit en signaleert crisissituaties.
Je draagt bij aan herstel en participatie in de maatschappij. Een BIG-registratie of HBO-opleiding is vereist.
