GGZ in Geest zoekt een casemanager voor een FACT-team voor de periode van een half jaar tot 1 januari 2027. Je beheert jouw eigen caseload, voert intakegesprekken en verzorgt huisbezoeken op locatie.
Je werkt systeem- en klachtgericht aan behandelingen en handelt crisissituaties zelfstandig af in samenwerking met een multidisciplinair team. Daarnaast ondersteun je cliënten bij herstel en participatie in de maatschappij en onderhoud je netwerken met andere organisaties.
